 Deploying a Board Matrix

Hello Community!

We want to deploy an electronic board matrix survey.  Does anyone have suggestions/recommendations for executing one efficiently? Ideally, the responses would automatically populate into a spreadsheet for evaluation and data analysis.  Has anyone used a Google product or Survey Monkey (or similar?) with success?

We use QuestionPro for our annual retrospective/feedback survey.  It does not exactly populate a spreadsheet but the results are easy to download.

And we just started using Google Forms.  This provide a lot of functionality and populates directly to a spreadsheet in Google Drive.
