I am curious to see what other museums have in the way of self evaluation forms used in replacement of/or in addition to performance reviews for museum staff.

HI - at the JFK Hyannis Museum, I present a set of goals to the board of directors at the start of the fiscal year - these are always tied to the strategic plan for that year. They approve the goals.  Then at self-review time, I plug in results and measurements, against those goals and give to the chair of the review committee.  It works pretty well, and everything stays aligned with the work.

We do a combination of a self-assessment and supervisors assessment for each staff. For me, as Executive Director, I do something similar to Wendy's process. At each performance review, I also set goals for the next year against which I measure myself in a self-assessment. In addition, each trustee completes an evaluation of my performance and the president of the board interviews staff members for their feedback.
