Think about your exhibit/program messages and goals and form questions to see if they were met for the visitors/attendees.

I'm interested in hearing about the kinds of questions one might ask in a visitor or program attendee survey that might be sent or given as a follow-up to a program such as an art lecture or workshop with the goal in mind to supply the NEA with information regarding the success of the program. I would be grateful to receive any sample surveys you've provided your attendees, either within this thread, or by email to me at olivia@sfcamerawork.org. I would also be interested in simple survey platforms that we might use on a static ipad in-gallery.
