Good Morning Everyone,
The Museum of the American Revolution is hosting a one-hour event for children 9-13 entitled "Write the Revolution." It will be hosted on a Sunday afternoon. After the event, the parents are invited to take part in a discussion, along with their children.
We simply need a Waiver Form for the hour the parents will not be with their children. Since we are located in a central part of the city, we assume most parents will stay relatively close for this one-hour period (maybe even explore the Museum on their own). All of the waiver forms we have found online are a bit too detailed and unnecessary for the type of event this will be. The main point of the waiver is to accept medical attention if the child should need it within the hour long program.
Any help of forms that you've used in the past would be fantastic!
