Hello all, 

Our governing organization is instituting a child waiver policy for all individual participants in program activities. Do any of your institutions have a policy or waiver, of such nature, for any free or fee based programs, not including separate camps? The thought is the child's parent or legal guardian would have to sign this before doing any programming. Definitions are still being defined at this time. 

Any models or information would be greatly appreciated!

Hi Megan,
We have a couple of different waiver forms we use when anyone --minors or adults -- are participating in our programming.  I've attached a copy of the most simple waiver of liability and photography release we use.  This form is part and parcel of our class registration paperwork and must be completed for any organized activity on our campus.  The photography release is also important for us because we often use photos in in our publications and marketing materials.  It alerts parents right away that we might use photos that include their child and gives them the opportunity to elect NOT to let us release their child's picture.
