Hi there, we are looking to implement a paper document for access outside each of our collections storage rooms. Do you have a sample document you don't mind sharing? I have created a template but I'm also wondering how other institutions utilize similar forms.
We are hoping to track:
Name/party of who is accessing the room, date, time, purpose, if objects were moved, where they were moved too, etc.
