    We would like to collect feedback from teachers who bring their classes to the museum for field trips. Would any of you be willing to share what form you use in your education programs? We would like to evaluate the presenter as well as the program. Thanks!

    ​Hi John,
    We actually created an evaluation on Survey Monkey. We give teachers and chaperones collateral when they visit or when we go to them has the survey link. We also send the booking teacher/administrator an email after their visit that includes the link to the survey. The survey is built in a way that their answers about what they did (came to us, had a program here, had a program at their location) determines what questions they answer next. I'd be happy to share more details about how it works, the questions used, etc.

    We also use Survey Monkey to collect feedback from our teachers and other group leaders. Surveys are sent the Monday following their visit and ask them to rate their experience, tell us what they liked or disliked, and offer suggestions for ways we can improve.

    We were frustrated by a couple of 3rd party evaluation services and ending up drafting our own Formstack survey that is sent a day or so after their program. Sample below.

    ​(I didn't notice if other posts already mentioned this)
    At AAM, a couple presenters suggested that, when providing an evaluation range, say of 1 to 5, have 4 be "Excellent" and 5 be "Outstanding" or "Superior."  There have been some studies that indicate that doing so will return a more useful snapshot, than the typical bimodal distribution.

    We give paper surveys with a self-addressed, stamped envelope. We have a pretty good response rate. I think most teachers fill them out on the bus ride home and then drop them in the mail. We haven't had a good response with electronic surveys.

    The feedback from here is printed on a half sheet of card stock with our address on the other side and a place for postage.

    These are handed out at the end of a tour, and we ask our Guide or Docent to encourage the teacher/chaperone to complete it on site and leave it at the front desk on the way out. 

    A VERY short form given immediately after the tour yields a very high return rate. On-line surveys are easier for staff to manage, but I doubt as many get completed. Also, with an on-line form, only the lead teacher/organizer gets the form. With ours, every teacher/chaperone with 15 students gets one.

    We can see around 60,000 K-12 students on guided tours, so it's a lot to keep track of, but I think it's worth it.
