We're planning a visitor input kiosk for an upcoming exhibition. We will use iPads with keyboards. I'm looking for an app that can run offline to collect visitor comments and stories. I'd like to be able to change the prompt and to curate a selection of the feedback to project onto the wall.

    Any recommendations?

One caution on automatic display of visitor input is the inevitable input by a (typically) young visitor who intentionally wrote something unsavory.  At Experience Music Project, we had a video room where visitors could leave a short video describing their first concert. We quickly realized we needed to hand pick the videos we wanted shown.

You might want to look up Brad Larson, who's been working on collecting stories from museum visitors for some time now.  bradlarson.com | visitor storytelling technology for museums  His StoryKiosks have won many awards and have been used in onsite museum settings, offsite popup and community settings.  He's a very nice guy and easy to work with.
